FMPH 495/499: Master's Capstone/Thesis Independent Study Courses
- Enroll in FMPH 495 the first quarter that you start working on your thesis or capstone project. Submit the following form to mphinfo@health.ucsd.edu no later than the first day of the quarter that you would like to enroll in 495:
- Enroll in FMPH 499 after successful completion of FMPH 495. Submit the following form to mphinfo@health.ucsd.edu by the Friday of 10th week in the quarter you are taking 495:

FMPH 498: Independent Study
MPH students who would like to earn credit for independent study work with faculty must submit an FMPH 498 Independent Study Request form. This form must be signed by the faculty member supervising the independent study, or come from their faculty supervisor's UCSD email.
Independent Study forms should be submitted at least 2 weeks before the start of the quarter in which the student is doing their independent study.
